Healthspan Physical Health Assessment Consent

 
Your Healthspan Physical Health Assessment appointment is an assessment with a trained kinesiologist as part of your Healthspan service. The assessment will include measuring your weight, height, heart rate, blood pressure, and VO2 max. 

A VO2 max test is a maximal exercise test. You will exercise on a treadmill or stationary bike while connected to a machine that can analyze your exhaled breath. If the kinesiologist determines that you are not safe for VO2 max testing (e.g., due to high blood pressure), they will let you know and you will not be able to complete that portion of the physical assessment. They may provide you with a letter that you can bring to a medical doctor or nurse practitioner for medical clearance to participate in VO2 max testing. In this case, you may rebook the VO2 max test at a later time without charge. 

You understand that physical activities involved in this appointment may be strenuous and may involve you doing movements and activities that you are not familiar with. You understand that you are responsible for monitoring your own condition during physical activity and letting the kinesiologist know right away if you feel unwell. Should you choose to engage in physical activity, you should stop immediately if you feel unable to continue safely or do not want to continue for any reason. If you feel chest pain, shortness of breath, dizziness, or other unusual symptoms during physical activity, you should stop and let the kinesiologist know, as well as seek out immediate medical attention. 

In the event of a medical emergency during the physical health assessment, you authorise the kinesiology clinic and its associated professionals to take all reasonable responsive actions, including contacting your emergency contact and emergency services to provide care. 

Privacy and Sharing 
You authorise the kinesiology clinic and its associated professionals to collect and view your relevant personal information for the provision of services. In addition, you authorise the clinic and its associated professionals to share your personal information with Healthspan Labs and your family doctor/nurse practitioner. As part of routine practice, the kinesiologist may occasionally consult with Healthspan and professional colleagues regarding practices, and when this is done, it is for the purpose of benefiting patients and personal information is kept confidential. 

Cancellation Policy 
Please give the kinesiologist at least 24 hours’ notice if you need to cancel or reschedule your appointment. Because the time you booked has been set aside and dedicated to you, it is challenging to fill the time on short notice. The kinesiology clinic cannot guarantee that they will be able to promptly reschedule a late cancellation or missed visit.

Electronic Communication 
The kinesiology clinic uses electronic communication, such as for sharing your information with Healthspan Labs. The clinic is careful to take reasonable precautions to safeguard your data when using electronic communication; however, just like social media and online shopping, electronic communication has some inherent privacy and security risks that your personal information may be intercepted or unintentionally disclosed, which can not be completely protected against. 

COVID-19 
Any in-person treatment involves some risk of COVID-19 transmission. You will let the kinesiology clinic know before any in-person services if you have COVID-19 symptoms or have tested positive for COVID-19 within the 5 days leading to your appointment; and it is reasonable that they cancel or reschedule your appointment if this is the case. You, and anyone else present, are recommended to wear a face covering during the appointment; however, face coverings will need to be removed during VO2 max testing.
